Upon submitting a request, you will receive an authentication email to the email address you have specified in this form. Please follow the instructions in the authentication email to complete your request. In order to verify your identify, we may request that you match specific pieces of information you have provided us previously, as well as, in some instances, a signed declaration under penalty of perjury that you are the consumer whose personal information is the subject of the request. If you are an agent acting on behalf of a consumer, we will require proof that you are authorized to act on their behalf and proof of your own identity. If you do not follow the instructions in the authentication email within 45 days, your request will not be processed and you will have to resubmit your request.
